3. Assignment if the applicant is not the inventor (simply signed, no notarization required)
4. Priority document (if priority is claimed)

For PCT National Phase application under Chapter I:

1. Power of attorney (simply signed, no notarization required)
2. Patent specification which includes description, claim(s), abstract, drawing(s) if any
3. Assignment if the applicant is not the inventor (simply signed, no notarization required)
4. Priority document (if priority is claimed)
5. Copy of International publication
6. Copy of International Search Report
7. Copy of PCT Request

For PCT National Phase application under Chapter II:

1. Same as number 1-6 under Chapter I
2. International Preliminary Examination Report
3. PCT Information Concerning Elected Offices Notified of Their Election (Form PCT/IB/332)
